Admission to Health Information Technology Program
Health Information Technology-Associate in Science
Completed applications are accepted year round on a first-qualified, first-applied basis.
Note: Students who fail to attend the term for which admission is granted must reapply to the program.
Application and Admission Requirements
- Completed PHSC application and application fee
- Completed Health Information Technology application and application fee
- Official high school transcript indicating graduation and receipt of standard high school diploma or an official transcript of GED scores indicating receipt of a high school equivalency diploma
- Official transcript from each U.S. college and university attended other than PHSC
- Official scores from external agencies as appropriate for prerequisite courses (ex: AP, CLEP, IB, etc.)
- Transfer credits: Courses transferred in may not match the PHSC course required to demonstrate prerequisites have been met. In cases where the transfer course is not equivalent to PHSC number, applicant must meet with advisor to request a "course substitution." Petitions to have a "course substitution" approved to meet prerequisites must be "approved" before application is submitted. Applications without approved substitutions are considered incomplete.
- Cumulative grade point average (GPA) of 2.0
- Completion of all of the following prerequisite courses with a combined GPA of 2.5 or higher with at least a grade of "C" or higher:
